Who can register a birth?
For married couples, either parent can register the birth on their own.
In the case of a child born to an unmarried couple, the name of the father may only be recorded in the entry of birth if both parents attend and sign the registration together or a declaration of paternity is produced.
It is the primary responsibility of the mother or father to register the birth. If this is not possible, the following people can register the birth-:

  • grandmother or grandfather
  • aunt or uncle
  • occupier of the premises where the baby was born
  • any person present at the child’s birth
  • any person who has charge of the child

What information do I need to bring?
You will need to fill in a birth registration form. This should be completed by the person registering the birth (normally the child’s mother or father). You do not need to bring any documentation with you.

Birth Registration Form

You will also need to provide the full name, surname and sex of the baby, the date and place of birth and the full names, addresses, dates of birth and occupations of the baby’s mother and father.
Please note that a registration cannot easily be changed when completed.

What will I receive?
When the birth is registered, you will receive one free short birth certificate and an HS123 infant registration form to allow you to register with your family doctor and obtain your child’s medical card. Additional certificates, either short or full, may be purchased for £8.00 per copy at the time of registration or £15.00 thereafter.
